What is Hospice?
Hospice is care for those faced with end-of-life conditions.  

Hospice includes not only the patient, but their families and caregivers as well.

Hospice treats the physical, emotional and spiritual needs of patients.

Hospice is provided where ever the patient resides; at home, in a nursing facility or in a hospital.

Hospice focuses on pain and symptom management, making the patient feel as comfortable as
they want to be so they can enjoy quality of life.

Hospice services are available to patients of any age, religion, race, or illness, regardless of their
method of payment.

Hospice team begins with the patient, their friends and family members.

Hospice plan of care is developed from the wishes of the patient, adding to this the experience of
physicians, nurses, social workers, chaplains, home health aides, and trained volunteers.

Hospice plan of care is specific to the patient's individual needs for care and support.

Hospice plan of care also includes medications, medical equipment, and supplies related to the
patient's hospice diagnosis.

Hospice provides grief support to the patient's family for at least 12 months.
Who pays for Hospice?
Medicare Part A   In most circumstances, Medicare Beneficiaries are eligible to receive hospice
benefits regardless of where the reside; at home, in a hospital or in a long-term-care facility such
as a Nursing Home or Assisted Living facility.

Medicaid   In most circumstances, Medicaid eligible beneficiaries can elect hospice benefits,
again, regardless of where they reside.

Private Insurance    Many private insurance plans include a hospice benefit.  If you are not sure, or
you believe your insurance may not provide a hospice benefit, contact your insurance provider or
hospice for assistance.

Private Pay    Even if you do not have conventional payer sources, please contact a hospice
provider and discuss your options.
What should I do next?
